1. Adult Family Care (AFC)
Wholistic provides Adult Family Care (AFC) in a homelike environment for up to four individuals at a time. Clients live with an unrelated caregiver who offers 24/7 care, including supervision, socialization, meal preparation, medication oversight, and hands-on assistance with daily activities such as bathing, dressing, and grooming. This service ensures that individuals receive continuous care while maintaining a level of independence in a safe and comfortable environment.
2. Structured Family Caregiving (SFC)
Structured Family Caregiving (SFC) is a caregiving service
arrangement in which a waiver participant lives together with
a principal caregiver who provides daily care and support to
the participant based on the participant’s daily care needs
and activities of daily living (ADLs).
3. Attendant Care (ATTC)
Wholistic provides Attendant Care (ATTC), offering hands-on assistance with Activities of Daily Living (ADLs), including personal grooming, dressing, meal preparation, and transfers. Our dedicated caregivers support individuals in their homes, helping them with day-to-day tasks while ensuring their safety and well-being. ATTC promotes independence by helping clients maintain their routines and personal care without skilled medical intervention.
4. Non-Medical Transportation (TRAN)
Wholistic provides Non-Medical Transportation (TRAN) to assist individuals in accessing community resources and services. We offer both assisted and non-assisted transportation options, ensuring that clients can safely travel to non-medical appointments, grocery stores, gyms, or other community activities. Our transportation services provide reliable, accessible options to keep clients engaged and active in their communities.

What is AFC?
Adult Family Care (AFC) is designed to offer 24/7 care in a homelike setting for up to four individuals. Under AFC, individuals live in the home of an unrelated caregiver who provides supervision, meals, socialization, medication oversight, and hands-on assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). This model allows individuals to maintain their independence while receiving essential support.
Services Include:
Assistance with ADLs (e.g., bathing, dressing, and grooming)
Supervision for safety and medication oversight
Meal planning, preparation, and cleanup
Socialization and companionship
What is Required to Provide AFC?
A safe, habitable, and comfortable home environment free of hazards
Privacy and opportunity for the individual to personalize their living space
Care provided by an unrelated caregiver (no family, POAs, or legal guardians).
AFC provides care 24/7, ensuring that individuals are never left alone. Each home must provide an environment that promotes the individual’s safety, privacy, and comfort.
Attendant Care
Available Under Pathways, Health and Wellness, and TBI Waivers
What is ATTC?
Attendant Care (ATTC) offers direct, hands-on assistance for individuals with their daily functional needs, allowing them to remain independent in their homes. The focus is on helping individuals perform ADLs, such as bathing, dressing, grooming, and meal preparation. ATTC also provides medication reminders and transportation to non-medical activities in the community.
Services Include:
Assistance with ADLs (bathing, grooming, dressing, transfers, ambulation)
Meal planning, preparation, and cleanup
Medication reminders
Safety monitoring
Transportation to community activities (non-medical)
What is Needed to Provide ATTC?
Providers must be licensed by the state health commissioner and meet the following criteria:
Plan for managing unstable health conditions
Employ a personal services agency manager
Complete client service plans and satisfaction reviews
Comply with client rights regulations and complaint investigations
Ensure caregivers complete TB testing, education, and training.
Please note that ATTC does not include skilled care services, such as wound care or specialized feedings, which require licensed health professionals.
Supporting the waiver recipient with activities of daily
living such as personal care assistance, including activities
such as bathing, oral hygiene, hair care, shaving, hand and
foot care, intact skin care, application of cosmetics,
dressing, toileting, eating, etc.
Supporting the waiver recipient with Instrumental
Activities of Daily Living such as meal preparation and
housekeeping
Transportation to non-medical community activities which
exceeds assistance required for an individual meeting
developmental milestone of the same age.
● Mobility assistance
● Health and safety and household tasks, such as making
the member’s environment free from safety risks, providing
reminders to take medicine, and assisting with
correspondence, or paying bills.

What is TRAN?
Non-medical transportation services enable individuals to travel safely to community activities, services, and resources such as gyms, grocery stores, and community centers. At Wholistic Home Care Services, we provide both assisted and non-assisted transportation options.
Types of Transportation Services:
Non-Assisted Transportation: For individuals who do not require assistance or a lift to get in and out of the vehicle.
Assisted Transportation: For individuals who need a lift to transfer in and out of the vehicle.
What is Needed to Provide TRAN?
Valid vehicle registration and insurance
A current driver’s license
Drivers must be at least 18 years old
Our transportation services ensure that individuals have access to the community and essential services, promoting their independence and active engagement.
STRUCTURED FAMILY CAREGIVING
Available Under Pathways, Health and Wellness, and TBI Waivers
Supporting the waiver recipient with activities of daily
living such as personal care assistance, including activities
such as bathing, oral hygiene, hair care, shaving, hand and
foot care, intact skin care, application of cosmetics,
dressing, toileting, eating, etc.
Supporting the waiver recipient with Instrumental
Activities of Daily living such as meal preparation &
housekeeping.
Transportation to appointments and community activities
that are therapeutic in nature or assist with maintaining
natural supports.
Mobility assistance
Accompanying the recipient to necessary appointments,
whenever possible, such as transporting individuals to the
doctor
